Do you have a group that’s ineligible for Delta Classic PPO plans due to their type of business? Delta Choice dental plans are exclusive to CoPower:
- There are no excluded Industries for PPO plans on the Delta Choice portfolio
- There is a minimum of 5 enrollees
- The employer contribution is always 100% for employees (100% participation) and 50% for dependents on all choice plans
- Delta Dual Choice plans can’t be both a Delta Choice plan and a Delta Classic plan. Your group would have to do a PPO/HMO Choice or a PPO/HMO Classic plan
- Deductibles are waived for diagnostic & preventive services ($25 individual, $75 family)
- For product combinations, Delta Choice plans require a minimum enrollment of 10 eligible employees (at least 3 enrolled in one plan and the balance in the other)
- All Delta Choice groups with effective dates of January 1 – June 1 have a January 1 renewal date
- All Delta Choice groups with July 1 – December 1 effective dates have a July 1 renewal date
